I pledge and promise to donate for every birdie made during the 2010 FBR Open to the Birdies for Charity Program. You may also specify a one time donation instead (please make check payable to THUNDERBIRD CHARITIES).

If 1500 birdies are made, my total pledge would be:
$0.01 = $15.00	$0.04 = $60.00
$0.02 = $30.00	$0.05 = $75.00
$0.03 = $45.00	$0.10 = $150.00

	One Time Donation	$ ($20.00 minimum)
